HCHC’s Care Management provides a variety of services based on each individual client’s specific needs.

Telemedicine
Clinical support from providers at the University of Virginia in Charlottesville. Telemedicine visits currently take place at the VMRC Community Health Center.

HIV/AIDS Testing
HIV/AIDS awareness is a sensitive and vital topic for communities. We offer confidential, rapid HIV Testing at our Harrisonburg offices and other sites throughout the community. Click here for more information.

Food Pantry
Food insecurity is an issue that affects thousands of Americans. Accessing nutritious foods including fresh fruits and vegetables can be particularly challenging when living on a reduced income. In order to meet the nutrition needs of our clients, Care Management provides an on-site food pantry and also delivers food directly to clients’ homes. Thanks to grant funding and local donations, we are able to stock our food pantry with nutritious foods, paper products, and nutritional supplements, such as Ensure.

Housing Assistance
Housing is a basic human need that is crucial to maintaining one’s health. However, sometimes it can be difficult to access and maintain adequate housing. Your case manager will work with you to find the best solution to your housing needs, whether it be applying for a Section 8 voucher, searching for subsidized housing, working on budgeting issues, or transitioning out of a shelter. We are also able to assist in emergency situations with rent payments, utility payments, and security deposits. In addition to short-term emergency housing services, Care Management also runs a long-term, subsidy-based housing program for eligible clients.

Client-Centered Medical & Non-Medical Case Management
Together with your medical case manager, you will develop an individualized service plan that addresses all of your health needs. Care Management service plans focus on holistic health and well-being, not just an HIV/AIDS diagnosis. Medical case managers assist clients in working towards better health and self-sufficiency. As part of this process, your case manager will assist you in accessing specialized treatment for HIV/AIDS, primary medical care, dental care, and can also help you access mental health services, substance abuse treatment, nutritional counseling, and any other referred medical service. Your Medical Case Manager will help you navigate complex health insurance systems to ensure that you can access your treatments and medications in a timely manner.

Transportation
As parts of our service area are very rural and lacking adequate public transportation services, Care Management provides access to transportation to medical appointments. We ensure that clients can get to their medical appointments by providing cab rides, gasoline vouchers, and bus tickets.
