GENERAL DESCRIPTION OF ROLE AND DUTIES
Nurses (LPNs and RNs) are vital members of the Care Management team. Nurses provide a safe patient care environment, support all activities of the Care Management and health care team, care delivery and promote excellence and efficiency in customer service. Nurses at HCHC participate in all performance improvement initiatives and must be knowledgeable of required clinical outcome measurements. Nurses have an important role in achieving improved quality of care outcomes and patient volume benchmarks through support of physicians, nurse practitioners, and physician assistants. At Harrisonburg Community Health Center the Care Management Telemedicine/PrEP Navigator Nurse has a role in assuring that their position supports meeting and maintaining VDH and HRSA Ryan White standards. This is accomplished in part through the Care Management Telemedicine/PrEP Navigator Nurse demonstrating successful, professional nursing practices, patient care coordination, ethical accountability, and the preservation of patient and staff confidentiality. The Care Management Telemedicine/PrEP Navigator Nurse is responsible for accurate documentation of telemedicine/PrEP Navigator client interactions, program development and growth, in collaboration with Care Management Director, and reports related to telemedicine/PrEP Navigator program implementation, as identified by VDH Ryan White and HRSA standards.
QUALIFICATIONS AND REQUIREMENTS FOR POSITION
Current state licensure for LPN or RN. Current CPR (BLS) certificate is required. Previous experience in a medical office setting is preferred but not required. Hearing must be adequate to perform the essential functions of the job, such as: auscultating blood pressure, ability to hear soft voices. Speaking must be adequate to perform the essential functions of the job, such as: clearly communicating instructions, providing input on assessments and evaluations of patients, taking nursing history, relaying information to multiple personnel, patients and their families. Employee must be able to prioritize, have cognitive reasoning, able to mathematically calculate for drug therapy. Employee must be aware of diversity among patient’s ethnicity and the impact of patient’s cultural background.
This is a moderately active role which requires the ability to life or move up to 25 pounds, bending, squatting, sitting and standing as necessary. Must be able to lift, carry and handle equipment, supplies and other work site materials based on position duty requirements.
