Nurse Practitioner

Harrisonburg Community Health Center is seeking an experienced Family Nurse Practitioner who has an expansive scope of practice, which encompasses care for newborns through end of life; experience in women’s health services that includes LARC insertion, women’s health including prenatal care in low risk patients. We are seeking an FNP who is interested in working in a team based care delivery model that integrates behavioral health services into the delivery or primary care.

The role of Nurse Practitioner (NP) at HCHC is to provide high quality primary health care services to patients of the health center, utilizing a team based model to support the mission and vision of HCHC. The NP utilizes the health center supported electronic health record as a means of documenting and reviewing patient care services rendered or ordered. The NP is board certified or board eligible licensed practitioner. The NP is a member of the clinical team at HCHC. The NP, unless otherwise noted, does not have formal supervisory authority over other health center staff. However, the NP is expected to contribute to the ongoing knowledge enhancement of other clinical staff in order to achieve the best possible clinical outcomes for health center patients.

NP will work to achieve the access to care, quality metric and patient satisfaction goals established by the Health Center.

NP will be provided an employment agreement that may elaborate on specific activities such as participation in peer review, Quality Improvement initiatives, and other activities that respond to and ensure the long-term viability of HCHC.

Annual evaluations will encompass efforts that support the HCHC mission and team goals. The NP and HCHC will negotiate any agreement for medical practice outside of HCHC, providing such practice does not interfere with NP obligations to the health center. All NPs will work under the guidelines of the collaborating physician agreement.

This job description applies to any NP practicing in any location associated with HCHC and approved within the scope of project by HRSA. NP scope of practice is dependent upon field of specialty and experience. Pediatric Nurse Practitioners provide care for newborns through age 18-21, pending experience and scope.
