Appointments

Making Your Appointment
Appointments at the Harrisonburg Community Health Center are by appointment only. The Community Health Center is not a WALK-IN health center. Appointments can be made by calling 433-4913 (area code 540). If you are having trouble reaching us on the telephone, look at our Phone Guide. If you are more than 15 minutes late, your appointment may have to be rescheduled. First-time appointments are asked to arrive 30 minutes prior to their appointment to complete new patient information. The Community Health Center requires 24-hour notice of appointment cancellation. Repeated "No-Shows" for appointments can result in being dismissed from the practice.

When you make your first appointment at the Harrisonburg Community Health Center you have the opportunity to review all of the information and forms included in our Patient Packet in the Forms & Documents section of our website. You may want to read over them, fill them out to bring to your appointment, or have a friend go over them with you in advance. It is not a requirement that you bring your forms already filled out. We will always have a Patient Packet available to you and a staff member to go over the information with you if you have questions or need assistance with the forms.

Cancellation & No-SHOW Policy

If you have an appointment scheduled and you are unable to come, you will need to call the Harrisonburg Community Health Center at 433-4913 (area code 540) to cancel the appointment. Please make this call before the appointment time. If you are having trouble reaching us on the telephone, look at our Phone Guide.

How Do I Cancel My Appointment? If you need to cancel an appointment at the Harrisonburg Community Health Center, please call 433-4913 (area code 540). If you get voicemail, please leave your name, the date & time of your appointment, your phone number, and the reason you are canceling your appointment.

What Happens If I Miss My Appointment and Do Not Call to Cancel?

Effective August 1, 2009 Harrisonburg Community Health Center will enforce a No-Show (missed appointment with no prior notice) Policy.

As a patient of our Community Health Center you are asked to make all reasonable attempts to your keep your scheduled appointment. If you need to cancel an appointment we ask that you call 48 hours in advance.

As a courtesy to our patients we will call to confirm your appointment two days before your scheduled appointment. If you need to cancel or reschedule your appointment we ask that you call between business hours of 8:30 a.m. - 4:30 p.m. This notification allows us to offer the opportunity for services to another patient. If you do not call to cancel your appointment and miss your appointment that appointment will be considered a no-show visit.

Our no-show policy is:

First “NO SHOW” — you will receive a letter informing you of the no-show along with a copy of our no show policy. You will be able to continue receiving services at the Community Health Center.

Second “NO SHOW” — you will receive a post card reminding this is your second no-show.

Third “NO SHOW” — you will be dismissed from the Community Health Center for 90 days. Your treating provider will provide you with a 90-day supply of prescriptions if necessary. You will be offered assistance in finding another provider. During this time period you may call the Community Health Center regarding your medications only.

Fourth “NO SHOW” — you will be dismissed from the Community Health Center permanently. Medications will be provided for 30-days and you will be offered assistance in finding another provider.
